Waiver: I understand that Taylor Parks and Recreation activities have inherent risk and I hereby assume all risk and hazards arising out of my participation, or my child’s participation, in all such activities including transportation to and from activities. I further agree to waive, hold harmless, release, and indemnify the City of Taylor, Taylor ISD, its employees, volunteers, officers, directors, contractors, participants, coaches, referees, as well as persons or parents transporting participants to and from such activities, from and against any and all claims, costs, expenses, liability, property damage, or personal injury arising out of or related to the use of City of Taylor and Taylor ISD facilities or participation in any City of Taylor and Taylor ISD activity, whether located on City of Taylor and Taylor ISD property or not, by me or my child.

Compliance with Rules: I acknowledge and agree that failure to abide by City of Taylor and Taylor ISD rules or policies may result in my suspension or expulsion, or my child’s suspension or expulsion, from any activity, event, facility, and/or league.

Photo Release:  I understand that the City staff reserves the right to photograph and/or videotape facilities, activities and program participants for potential future use. I hereby grant permission to the City to use my or my participant’s photograph and/or video footage for any lawful purpose, including for example such purposes as publicity, advertising and website entries. I understand that I will not be paid or receive anything related to the City’s use of my/my participant’s photograph and/or video. I understand that all photographs and videos will remain the property of the City and I acknowledge the City’s right to alter or edit any photographs and/or videos at its discretion. I agree to release the City from any and all legal claims I or a third party may have arising from the use of my/my participant’s photograph and/or video footage.
